When creating a database initially, you specify filegroups and specific files as subclauses
to the CREATE DATABASE statement:

This CREATE DATABASE statement creates the primary filegroup??”the primary filegroup
is named, aptly enough, PRIMARY??”and two additional filegroups, each containing a single
file initially. Notice from the names of the filegroups that the intent here is most likely to
separate clustered and nonclustered indexes for performance purposes. Also note: The
log file has no direct link to any particular filegroup.
